Which shampoo ingredient is making your scalp itch?

Most likely one of three things: a preservative, a surfactant, or a fragrance material. They behave differently, they appear in different places on the label, and only one of them is what people usually blame. Sorting out which is which is what makes the list on the back of the bottle useful.

An itchy scalp after washing has two broad explanations, and telling them apart matters more than any single ingredient does. Irritant contact dermatitis is a dose-and-time effect: strip the skin barrier hard enough or often enough and it complains. Anyone can get it, it is worse with hot water and frequent washing, and it usually eases when you wash less or switch to something milder.

Allergic contact dermatitis is different. It only happens to people whose immune system has been sensitised to a specific molecule, it can appear days after contact, and once established it does not improve with dilution — a sensitised person reacts to trace levels. DermNet is clear that patch testing is the way this gets identified; you cannot deduce it from a bottle that disagreed with you, because a finished shampoo has twenty ingredients in it.

What the ingredient list can do is narrow the field before you get to a dermatologist. Here are the three groups worth knowing by name.

Methylisothiazolinone: the preservative with a date attached

If your shampoo is more than a few years old, or came from outside the EU, methylisothiazolinone is the first name to look for. MI is a preservative that became extremely widespread in the 2000s and then caused what dermatologists openly described as an epidemic of allergic contact dermatitis. The American Contact Dermatitis Society named it Contact Allergen of the Year in 2013.

The regulatory response was unusually decisive. The EU Scientific Committee on Consumer Safety could not demonstrate any safe concentration for leave-on products, and Commission Regulation (EU) 2016/1198 banned MI outright from leave-on cosmetics — creams, lotions and wet wipes included — applicable from 12 February 2017. Commission Regulation (EU) 2017/1224 then cut the permitted level in rinse-off products to 0.0015%, or 15 parts per million. The related MCI/MI mixture had already been restricted to rinse-off use by Regulation (EU) No 1003/2014.

So a shampoo sold in the EU today may still contain MI, legally, at a very low level. For most people that is fine. For someone already sensitised, DermNet notes that even those low rinse-off levels can trigger a reaction, and reactions in highly sensitised people have ranged from localised scalp or hand dermatitis to widespread and even airborne dermatitis.

The practical point: MI is one of the few ingredients where the year and the market a product came from genuinely change the answer. A bottle from the back of a cupboard is not covered by the 2017 rules.

Cocamidopropyl betaine: the allergen that mostly is not

Cocamidopropyl betaine — CAPB on a label — is the ingredient people find when they start searching, because it is in almost everything that foams: shampoo, shower gel, facial cleanser, baby wash. It is a coconut-derived amphoteric surfactant, and it is usually added to make a formula gentler, by softening the harshness of stronger detergents like sodium lauryl sulfate. A published safety assessment found a wide margin of safety at up to 6% in leave-on products and 30% in rinse-off ones.

It was also named Allergen of the Year by the American Contact Dermatitis Society in 2004, which is why it turns up on every "ingredients to avoid" list. Both of those things are true, and the reconciliation is genuinely interesting.

The evidence points to manufacturing impurities rather than CAPB itself as the main sensitisers — specifically amidoamine and 3-dimethylaminopropylamine, usually written DMAPA. People who patch-test positive to amidoamine or DMAPA are advised to avoid CAPB-containing products, but better-purified CAPB is expected to carry a lower sensitising risk. The label says "Cocamidopropyl Betaine" either way; it does not tell you how clean the batch was.

That nuance changes what you should do with the information. "CAPB is an allergen, avoid it" is a poor rule, because CAPB is often the reason a product is mild in the first place, and removing it usually means a harsher surfactant takes its place. "If you have patch-tested positive to CAPB, amidoamine or DMAPA, avoid it" is the rule that actually holds.

Fragrance, and why it sits in a different category

Fragrance is the most common cause of cosmetic allergic contact dermatitis overall, and it is handled differently on the label. Under Regulation (EC) No 1223/2009 a set of individual fragrance allergens must be declared by name rather than hidden inside "Parfum" — which is why you see Limonene, Linalool, Geraniol, Citronellol, Citral, Coumarin or Eugenol tacked onto the end of an ingredient list.

Limonene is the one people ask about most, and it has a wrinkle worth knowing. Pure limonene is only a weak sensitiser; it is the hydroperoxides it forms on exposure to air that DermNet describes as much more potent sensitisers. DermNet reports that around 10% of people investigated for suspected contact dermatitis test positive to the hydroperoxides of limonene and linalool. The practical consequence is that a product you tolerated when it was new can start causing trouble months after opening — the same bottle, oxidised.

This is also why the three groups need separating rather than merging into one avoid-list. A fragrance-allergy profile should flag limonene and not CAPB. A sensitive-skin profile has reason to flag both. Ingredients get flagged for what they actually are, not for a general sense that they sound risky — which is the same principle that keeps lanolin off the fragrance-allergen list despite being a well-known contact allergen in its own right.

And if none of the three names appear on your bottle, that is informative too. Frequent washing, hot water and a high-foaming formula are enough on their own to produce an irritant reaction in skin that is already dry or eczema-prone, with no allergen involved at all.

Frequently asked questions

Usually not — it is one of the milder surfactants and is often added specifically to soften harsher cleansers, so most sensitive skin tolerates it well. The exception is a known patch-test allergy to CAPB, amidoamine or DMAPA, in which case it should be avoided. A dermatologist’s patch-test result is the authoritative guide, not the ingredient list alone.

Because the sensitisers appear to be manufacturing impurities — amidoamine and DMAPA — rather than cocamidopropyl betaine itself, and better-purified CAPB is expected to carry a lower risk. In leave-on cosmetics in the EU, yes — Commission Regulation (EU) 2016/1198 prohibited it there, applicable from 12 February 2017, because no safe concentration could be demonstrated. It remains permitted in rinse-off products such as shampoos and shower gels at a maximum of 0.0015% (15 ppm) under Commission Regulation (EU) 2017/1224.

In a sensitised person, yes. Allergic contact dermatitis does not depend on dose the way irritation does, and DermNet notes that even the low levels permitted in rinse-off products can trigger reactions in people already sensitised to methylisothiazolinone. If you have a confirmed MI allergy, the standard advice is to avoid all MI-containing products.

SLS is a strong detergent used as a benchmark irritant in patch-testing research precisely because it reliably disrupts the skin barrier, and frequent use can leave skin dry and reactive. That is a different mechanism from allergy, and it responds to washing less or choosing a milder formula.

It means no declared fragrance allergens, which removes the most common cause of cosmetic contact allergy but not the others. A fragrance-free shampoo can still contain methylisothiazolinone, cocamidopropyl betaine and sodium lauryl sulfate. "Unscented" is weaker still, since it can mean a masking fragrance was used to cover a base odour.
