Lactose intolerance is a digestive sensitivity to lactose, the natural sugar in milk. Symptoms are uncomfortable but not dangerous, and many people tolerate small amounts. A cow's-milk allergy is an immune reaction to milk proteins — chiefly whey and casein — and can, in severe cases, be serious.

Why “lactose-free” is not “milk-free”
This is the trap. A “lactose-free” product has had its lactose removed or broken down with the enzyme lactase — but it still contains milk protein. For someone who is lactose intolerant, that's a good match. For someone with a milk allergy it is not safe: the whey and casein that trigger the allergy are still there.
So the two profiles read the label differently. Lactose intolerance is about the sugar; a milk allergy is about the protein. “Lactose-free,” “low-lactose” and lactase-treated claims answer the first question and say nothing reassuring about the second.
Where lactose and milk hide
Neither hides only in the dairy aisle. Milk and lactose turn up in bread, cured and processed meats, flavoured crisps, chocolate, biscuits, sauces, margarine and protein powders — listed as milk, milk powder, whey, casein, milk solids or simply lactose.
Because milk is one of the EU's 14 declarable allergens under Regulation (EU) No 1169/2011, any milk-derived ingredient must be emphasised in the ingredient list. How the symptoms differ
Timing is the clearest everyday signal. Lactose intolerance is a digestive problem: bloating, wind, cramps and diarrhoea that build up over roughly thirty minutes to two hours as undigested lactose ferments in the gut. It is dose-dependent, so a splash of milk in tea is often fine when a glass of it is not.
A cow's-milk allergy is an immune reaction and usually starts faster — minutes to about two hours — and can involve the skin and airway as well as the gut: hives, swelling, vomiting, wheezing and, in severe cases, anaphylaxis. It is not dose-dependent in the same way. Any of this is a reason to see a clinician rather than to self-diagnose from a label.
